During the inspection, the infant room had 1 caregiver with 5 children. The specified age group was 0-11 months. The caregiver was over in ratio by 1 child.

Based on the informatio obtained, trash was not being taken out regularly to avoid offensive odors in the classroom.

Based on the information obtained, there is sufficient evidence to show an additional caregiver was not added to a classroom where children were experiencing multiple issues with biting.

Observed glass cleaner in an unlocked cabinet in the classroom. The locking mechanism to the cabinet was broken.

Based on the information obtained during the investigation, the operation failed to report an incident that placed an infant at risk.

Based on the information obtained during the investigation, a caregiver placed an infant face down in the crib, for fifteen minutes.

The operation did not have the most recent sanitation report available to review, during the inspection.

In the infant room, I observed one infant sleeping in a crib with a pacifier in the mouth with an attachment adhered. Also in the crib was a burping cloth.

Based on the information obtained during the ongoing DFPS investigation, a caregiver placed a child outside of the classroom for crying.

Based on the information obtained during this investigation, the caregiver did not demonstrate patience or understating when a child was upset.

The fire inspection expired March 11,2021 and requires annual renewal.

A plan for personal contact with parents to exchange/provide observations, comments, and concerns was not adequately utilized. There was a lack of communication with parent(s) regarding ongoing behavior concerns for one toddler in care, which resulted in incidents involving other children.

A toddler's soiled diaper was not changed promptly.
