A child received stitches on their finger after they placed their hand inside of a padding that covered a pole on the playscape. The poles are covered by a padding and are secured by plastic lock ties. There was an opening in the padding where the child was able to put their hand in.

This standard was tasked as part of an investigation and found deficient. A 17-year-old caregiver attending high school is not enrolled in and has not completed a child-care-related career program.

Two toddlers were wandering alone in the operation on two different occasions.

A child was hit with a ball by another student while in care. As a result of this, a tooth that was loose fell out. The child's mouth was bleeding and required an oral rinse. The parent was not notified of this incident.

The caregiver in charge fail to supervise when a two year old was locked inside a supply closet for seven minutes.

The staff failed to make sure an area that was accessible to children was free from hazards. A two year old had access to glitter unsupervised.

A infant was observed in a bouncer near the front desk not being supervised.
