The operation allowed a child under 12 months to have a blanket in a crib to prop a bottle while the child was asleep.
At the time of the inspection, two children who no longer attend the center did not have immunization information in their storage record.
The operation has not been documenting daily sheets for any infant in care.
The operation did not document an illness on an illness report form after becoming aware that a child had been hospitalized after being in their care. The program was aware that the child did not feel well earlier during the day.
Caregivers were aware that a child was hospitalized for a medical illness and did not report it to Regulation.
This rule was evaluated with regard to an investigation and a deficiency of the Minimum Standard Rule was found. Based on the observations, it was observed that boxes were being stacked on the floor in the gym and the gym is used for the children and these boxes could fall onto the children. In addition, there is a broken door observed that was coming from one of the restroom in the gym.
At the time of the inspection, three children with noted food allergies did not have completed food allergy plans.
At the time of the inspection, a caregiver did not have current CPR training.
At the time of the inspection fire extinguishers throughout the center were not mounted.
At the time of the inspection, one child did not have an emergency allergy plan signed by a health care professional.
At the time of the inspection televisions were not anchored in two classrooms.
At the time of the inspection the playgrounds did not have the proper depth of loose-fill surfacing.
