Operation did not have attendance and time sheets for director and caregiver.

The operation was not following their daily schedule. Children engaged in free play and the operation did not follow posted activities.

The operation did not have a copy of photo ID in 1 of 2 personnel records reviewed. This was corrected at inspection when the caregiver printed the item and placed in file.

An infant in care did not have updated infant feeding instructions.

There was no record available for 1 of 2 caregiver files requested.

It was observed that a caregiver was supervising a combined age group of 13 children during naptime. The youngest child in the group was 1 month old and the oldest child in the group was 7 years old.

The operation still did not have a current building and grounds checklist available for review.

There was no handwashing sink the the diaper changing area in an infant room. The operation removed it. The sink was reinstalled at the operation during this inspection.

It was observed that the operation had drain cleaner on the bottom shelf in the hallway of the operation that was accessible to children in care. This was corrected at inspection when the director removed the drain cleaner from the shelf and out of reach of the children in care.

The trash cans in the rooms where children were located were low to the ground and did not have lids making the garbage accessible to children in care.

A one month old infant was observed asleep in a baby rocker and swaddled inside of a blanket during the inspection. The infant was removed from the rocker and the blanket during this inspection.

The operation grouped children together in a toddler class that had more than an 18 month difference in age. The youngest child in the group was one month old and the oldest child in the group was 24 months old.
