An infant caregiver was observed placing a used baby bottle filled with liquid in a sink used for hand washing after diaper changes. This was corrected during the inspection when the bottle was removed and a "dish only" sign was placed above the sink.

This was corrected during the inspection, when the bucket was removed and the water supply to the water fountain was turned off.

Based on information obtained during an investigation, caregivers failed to properly supervise a child in care on two separate occasions. The child received a scratch and two child size bite marks at the operation and staff were not aware of when the injuries occurred. The caregivers are no longer working at the operation.
