A 3 month old infant was observed sleeping with a blanket and a 6 month old infant was observed sleeping with a bottle. The blanket and the bottle were removed from the crib.
Two infants aged 3 months were observed sleeping in restrictive devices.
The operation's last fire inspection was conducted in January of 2022.
A walker with wheels was observed in the infant room. The walker was removed from the operation during the inspection.
The fire extinguisher in the two year old hallway has not recently been serviced and the needle is in the red.

This standard was reviewed as part of an investigation and found to be deficient. It was determined that a caregiver hit children while in care.
The director and two van drivers do not have the required annual transportation training.
The first aid kits on the vans were missing cotton balls, scissors, thermometers, and tweezers.
The platform on the outdoor playground area does not have six inches of loose fill surfacing underneath. Also, the play structures does not have six feet of loose fill extending outward.
The brown van used for transportation seats were observed to be damaged and need repair.
Out of the five staff files evaluated four did not have 24 hours of the annual required training.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
One infant under 12 months of age were observed sleeping in a crib with a bottle in mouth during the inspection. The deficiency was corrected at inspection by removing all items from the crib.
There were no infant daily logs created for the children in care at the time of inspection.
