The operation did not reported an incident that put a child in a potential harmful situation in a timely manner.

Children under age of 18 months were over ratio. There were 8 children with 1 caregiver when there should have been two caregivers or 5 children.

The operation had several classrooms that did not have electrical outlet covers were children under 5 were present.

The operation did not report that a child was left unsupervised.

During the investigation it was determined that two classes were out of ratio. One teacher had 18 three year olds when the max allowed is 15. Another teacher had 14 two year olds when the max allowed is 11. The operation brought in additional staff to correct the ratio problem.

A child was left unsupervised in a classroom.

Caregivers was using their electric devices (cell phone) for personal use.

A caregiver used harsh and unusual punishment when she pulled a child by the feet.

A child was not allowed to participate in active play.

An infant in care did not have an updated feeding schdule.

A caregiver did not have transportation trainig and was allowed to transport children in care.

A child was enrolled in this program without complete admission forms and records.

Two children who were indicated to have food allergies, did not have food allergy emergency plans.

This standard was evaluated in regard to an investigation and a violation was found. The operation did not report positive cases of COVID to licensing within 2 days.
