One of the two home screenings reviewed did not have an interview for a household member.

One caregiver was missing the required safe sleeping training.

While reviewing background checks, there were multiple individuals listed as active that are no longer associated with the operation.

The operation has been operating since July of 2023, the background check validation has not been completed.

A family member living in the home was not interview. The interview was conducted over the phone with the family member on 12/6/2023.

The home-screening did not discuss health status of each person living in the home.

The home screening did not mention the attitude of each household members.

A caregiver left a weapon (taser) in the car which was accessible to children in care which could have resulted in physical harm.

This standard was found deficient as part of a DFPS Investigation.

Foster parent left children unsupervised, and this was not in accordance with their service plans.

There was one bedroom that was locked and foster mother stated that she did not have a key to the room.

There were multiple smoke detectors that were beeping upstairs and downstairs

There was prescription and over the counter medication in the master bathroom and one of the daughter's bedrooms.

There multiple blinds that had broken slats. This included the bedrooms and living room.

There was not a fire extinguisher on the second floor of the home.

The foster parent was utilizing a room that did not have a door for privacy. This was used for the foster child bedroom.

During the investigation, it was observed that a child in care was using a common area room as a bedroom without a door.

The foster home did have German roaches.

A child in care was tased by a foster parent and required medical attention. Foster parent had taser easily accessible to children in care.

Foster parent had taser within reach of the children in care in the vehicle.

Staff must have children files with the current documentation in the child's file. Three of the three child files read did not have the documentation of admission paperwork in file.

On 1/3/2024, the agency was not open during regular office hours.

This standard was found deficient as part of a DFPS Investigation.

Trampoline did not have absorbing pads covering springs.

Fire extinguishers was outdated and had not been checked since the family moved in the home.

Medication storage consisted of one lock on the closet door but there was not a second lock for psychotropic medications.

2 children reported the foster parent yelling at children in care

2 children reported being left at home alone without foster parent supervision.

The home was in need of repairs. The areas requiring repairs were the bathroom mirror, dining room chairs, and light fixtures.

A caregiver was observed, with the children, taking them through the backyard to enter the home. The caregiver does not know the alarm code. The caregiver enters the home through the unlocked sliding doors to prevent the alarm from going off.

The placement agreement and medical consent forms documents the name of a foster parent as being authorized to provide care. The authorization was signed by a respite provider.

Several food items were observed to be uncovered and spoiled in the refrigerator.

The verified foster parent moved out of the location of the foster home. The agency left the home's location open as an option for foster care placement.

An unverified respite provider accepted children into placement and signed paperwork in lieu of a foster parent who had no intentions of returning to provide care.

The operation was concerned that a caregiver was abusive towards a child in care and did not make a report to State Wide Intake.

The refrigerator contained several uncovered vegetables that had rotted and began to soil the surface of the storage bin.

Two children in care were placed in respite on 11/08/2023 and were still in respite placement on 11/29/2023.

One out of two personnel files reviewed did not include a signed affidavit for applicant. All personnel files need to have required documents in file.
