A caregiver was observed, with the children, taking them through the backyard to enter the home. The caregiver does not know the alarm code. The caregiver enters the home through the unlocked sliding doors to prevent the alarm from going off.
The placement agreement and medical consent forms documents the name of a foster parent as being authorized to provide care. The authorization was signed by a respite provider.
Several food items were observed to be uncovered and spoiled in the refrigerator.
The verified foster parent moved out of the location of the foster home. The agency left the home's location open as an option for foster care placement.
An unverified respite provider accepted children into placement and signed paperwork in lieu of a foster parent who had no intentions of returning to provide care.
The operation was concerned that a caregiver was abusive towards a child in care and did not make a report to State Wide Intake.
