Based on interviews and information gathered for this investigation, it was determined that caregivers did not provide adequate supervision on the playground. In two separate incidents, caregivers were unaware that they were missing a child they were responsible for. They had no visual/auditory awareness, they were unable to intervene, and they did not use a reliable method to account for the children they were responsible for.

A caregiver did not demonstrate good judgment when they inappropriately wrapped a child in their sleeping equipment so that they would not move off their mat. The caregiver also wrapped the elastic strap attached to the sleeping equipment around the child's legs to confine them.
