During the inspection a caregiver was caring for five children ages-6 months, 9 months, 10 months, 12months, and 15 months. This made the specified age group at 10 months causing the caregiver to be out of ratio. The ratios for this age group are 1 teacher per 4 children. This was corrected at the time of the inspection when another caregiver entered the classroom.
A teacher was in the one-year-old room caring for 4 children. When asked how many children she had she said 5. She had an infant in the infant room sleeping in a crib alone with no caregiver in the room. She said she thought both rooms were one room. This was corrected at the time of the inspection when the child was moved to the one-year-old room and another caregiver came to help in the room.
A caregiver's purse was accessible to children. The caregiver corrected this at the time of the inspection by moving the purse to a location that was not accessible to children.
Based on the information gathered during the inspection through interviews and viewing the video footage it was found that children were left unsupervised in the infant room, the one year old room and the two year old room on several occasions for unknown amounts of time.
During the inspection and also viewing the video footage it was found that the caregivers were not using the safety straps in the seats at the table nor on the highchairs.

Based on the information obtained there was enough information obtained that the playground was not being inspected daily for any hazards.
There were diaper rash creams and two other medications in children's cubbies that were not labeled with the child's name.
