Based on the information obtained during the investigation, there is sufficient evidence to verify that caregivers did not maintain visual awareness of each child?s ongoing activities. Caregivers did not intervene when children received unexplained injuries on various occasions.

Based on the information obtained, it was determined child was unsupervised for brief moment when he went into an empty classroom after returning from playground.

Upon arrival 3 out of 9 infants were asleep in restrictive devices. This was corrected when caregivers and owner removed the infants out of the restrictive devices and placed them in their cribs.

During the time of inspection a caregiver caring for 18 children was yelling at children while they were transitioning to use the bathroom and rest for nap time. Inspector and Owner was able to hear the care giver's voice from down the hallway and immediately walked in the room. The behavior was corrected when the inspector and owner walked in the room.
