The operational policies outdoor policy was lacking the benefits of playing outside. The policy does not address clothing or shoes the children should wear to safely play in the playground.

A caregiver conducting transportation picked up children at a school and did not utilize the transportation list. The caregiver accidently picked up a child that did not attend the center.

The Director left the center knowing that a classroom would be out of ratio.

A classroom did not have a working A/C and had a hole in the ceiling due to the repair fo the A/C.

A child was observed in the hallway unsupervised. The caregiver from the toddler classroom was observed out of the classroom not supervising the children.

The school age room had a total of 38 children with one caregiver.

The mulch in the playground is not six inches.

The school age classroom had a total of 39 children with two caregivers during naptime.

The transportation training did not contain the date the training was taken. Corrected at Inspection: The date of the training was added to the certificate.

Several children were observed sleeping on the floor with no blanket or mat to sleep on.
