A caregiver allowed children under two years old to watch a show on their phone.

The center does not have a current fire inspection.

The owner nor a director has signed the CPSC annual certificate.

Children were observed using the nap mats as a fort and laying on the mats and then the same mats were used for naptime. The mats were not sanitized between uses.

The emergency preparedness plan is lacking the evacuation and relocation diagram.
