Two out of ten children's records reviewed did not have the parent signature under the emergency medical care section.

An 8 month old infant was observed in a rocking bouncer and both the child and the rocker were in a play yard. Corrected at inspection: The child and the rocking bouncer was removed from the play yard.

The fire inspection was last conducted on 05/27/2021.

The infant diaper changing station had a mat that was torn and exposed the foam underneath.

In the toddler classroom they were watching two dinosaurs fight. It was not part of the curriculum for the classroom.

The director's certificate expired on 12/31/2021.

A fire drill was not conducted for June of 2021. Corrected at inspection: A fire drill was conducted during my inspection.

The director was lacking 28 hours out of the required 30 hours of annual training.

The center did not fill out a monthly checklist since the beginning of 2021. Corrected at inspection: The center has the checklist and filled out the checklist during the inspection.

The last health inspection was conducted on 11/15/2019 and expired in 2020.

Two out of four staff hours were lacking annual training hours for the 2019-2020 training year. One staff member only completed 15 out of 24 training hours. Another staff member only completed 6 out of 24 hours training hours.
