This standard was evaluated as part of an investigation and determined to be deficient. It was determined that a caregiver inappropriately grabbed and pulled a child by the arm.

This standard was evaluated as part of an investigation and found to be deficient. A caregiver smoked/vaped inside of a bathroom located inside the facility. The caregiver was terminated immediately after the administration was made aware of the incident.
