Two staff did not sign off that they have received orientation.

The school age playground has a broken platform. The border on the school age playground has the metal stakes coming out. The rubber surfacing on the school age play structure is breaking in multiple spots. The play structure has plastic pieces breaking off. The back area wooden fence has missing planks.

This deficiency was corrected at inspection when the director instructed a caregiver to move the correct level of wood chips in front of the slides use zones. The operation has ordered additional wood chips to keep the playground's loose fill properly maintained.

The operation does not have a current fire inspection.

A caregiver in room one did not wash an infant's hands after two diaper changes.

The current director has been in place since 12/27/2022 and has not provided director credentials to Licensing. The governing body did not inform Licensing until 02/10/2023 that a new director had been put in place.

The fencing around the playground has come separated from the top creating a safety risk to children in care. The trim on bottom of one of the classrooms is detached and in need of repair.

This was evaluated during the investigation and determined to be deficient. Operation was notified medical treatment was required on Friday 12/02/2022 and reported to CCR the following Tuesday 12/06/2022 going past the required two-day time frame.
