One caregiver was observed outside caring for 16 children with a specified age group of three years old.

Two children do not have immnization records on file.

The operation has not had a fire inspection since 9/2019.

Three caregivers did not know/were unsure of how many children were in care in their classroom during inspection.

It was determined through the course of an investigation that the operation the operations staff did not use good judgment when knowingly dismissing a child who's under clothes were soiled.

Based on information gathered through the course of investigation, children in the Three year old classroom were observed not washing their hands after outdoor activities, before eating, and after toileting.

Based on the evidence gathered through the course of investigation. A caregiver was observed forcibly grabbing three children by the arm and dragging them to different areas of the classroom. The caregiver was also observed forcibly grabbing another child by the arm. The caregiver was also observed forcibly sitting a child in a chair and forcing the chair under the table.

During the course of investigation it was determined that a caregiver left children unsupervised in the classroom multiple times in a 30 minute period. It was also determined that the caregiver allowed a child to exit the classroom onto the playground alone to retrieve an personal item.

A caregiver who has been employed longer than 90 days does not have 24 hours in pre-service training.

A caregiver does not have an employee affidavit on file.

Two children do not have Health Statements on file.

One employee is missing 23 hours of annual training.

Two children with food allergies do not have Food Allergy Plans on file.

The operation does not have records of recent or current playground maintenance checklists on file.

One center vehicle does not have a electronic child safety alarm installed and the safety alarm on another vehicle is not working properly.

Seven employees are missing annual training hours in the following topics: 1)Child Abuse/Neglect 2) SIDs,Child Brain Development,Shaken Baby Syndrome.

A child who has been in care over 12 months does not have a healthcare provider statement on file.

One employee does not have an Employment Affidavit on file.
