The safety plan that was identified for the children in care was not implemented to ensure the safety and supervision of the children. The agency immediately provided the alarms for the bedroom doors and had them installed. Pictures were received.

The supervisory visits did not have all indivduals present signatures on the form.

Fertilizer and gas containers were observed in the backyard in the children's play area

The foster parents did not report to Licensing when a child was injured by a glass table falling on their foot and fracturing a toe. The child did have to seek medical treatment.

The fence out door has a broken slate. There is a shed that appears to be unstable. There are three window screens that were torn and a window shade that appeared to be broken.

A foster parent yelled at children in care.

During the investigation inspection there were prescribed and over the counter medications not secured according to requirements.

A foster parent was observed using a shoe and their hand striking three children in care.

Two 2-year-old and a 3-year-old child in care were observed with multiple injuries to legs, torso, and neck. The foster parent was observed hitting children in care with a shoe and hand.

As part of an investigation home study and training log documentation were requested from the agency but never provided.

A child in care attempted to run away. A caregiver instructed a child to physically prevent this activity from happening.

A child's supervision required close monitoring and instructed that the child remain in the common areas when not sleeping. The caregivers left the child unattended in the foster home.
