The preschool 3/4 classroom had nineteen children with one caregiver. The specific age is four years old. The children were playing with various toys on the floor with the caregiver. The operation corrected the ratio during the visit by adding another caregiver to the classroom. The class was in ratio with two caregivers and nineteen children.
The cribs in the infant room had loose-fitting sheets. During the monitoring visit, the caregivers added bed sheet safety straps to all the cribs for infants in the classroom.
There were three infants in the restrictive devices from 12:17 to 12:45 pm. The caregiver removed one infant from the device at 12:39 pm., and the other two remained until 12:45 pm. The caregivers sat on the floor with the infants with small toys to play with. The ages were 7 months, 7 months, and 13 months.
The water hose was left outside on the toddler playground. 1 child was observed picking up the water hose.
After the caregiver helped the 2 year old children in the bathroom with toileting, the caregiver washed her hands but the children did not wash their hands.
The operation allowed a 14 year old volunteer to be present at the operation without a background check. The 14 year old was observed in the classroom with access to the children. This was corrected at inspection by having the 14 year old leave the operation.
Upon arrival the TV was on at the operation for the school age kids. The TV was still on after 1 hour, playing Mr. Beast Wipe Out.
This standard was evaluated during the investigation. It was determined that the operation was out of ratio in the 2's classroom, leaving 1 caregiver with 17 2-year-olds at once.
