Based off the DFPS investigation, the daycare was neglectful by leaving a child unsupervised for over 6 minutes which allowed the child to leave the center. The child then went through front door and went to the front of the operation where he walked down the road on a busy highway. The caregiver was unaware the child had left the center.

The operation did not have a suspension and expulsion policy, promotion of indoor and outdoor activity and services to include children with special care needs in operational policy.
After changing the diaper of one child the caregiver did not wash hands before reaching to pick up another child. This was corrected at inspection when licensing asked the caregiver to wash hands before picking up another child.
A classroom with children under 5 years old had 5 electrical outlets exposed with no covers. This was corrected at inspection when caregiver placed covers on all the outlets missing covers.
The operation failed to submit the number of employees that have left their employment in 2023 by January 15th.
The infant classroom had 5 children between the ages of 3 months - 9 months making the classroom over ratio by one child.
There was no one left in charge of the operation while a child locked themselves in a restroom.
Based on the DFPS investigation, school-age children were allowed to go back and forth to the bathroom without being supervised. Caregivers also left children unsupervised in the classroom.
Two out of four staff trainings that were reviewed did not have the 24 hours of annual training. One caregiver was lacking 4 hour of annual training for the 2021-2022 training year and was lacking 23 hours of annual training for the 2022-2023 training year. The other caregiver did not have any trainings for the 2021-2022 training year.
Two classrooms were observed to be out of ratio. An infant classroom had 6 children with one caregiver and the specified age group was 12-17 months. The classroom was one child over ratio. The second classroom had 15 two-year old children to one caregiver. They were 4 children over ratio.
The loose fill under surrounding the playhouse with the climbing wall and stairs does not have 6 inches.
Three drivers did not have a valid transportation training. The last transportation training was taken in December of 2021 and it expired in December of 2022.
The director was lacking 8 hours out of 30 annual training hours for the 2021-2022 training year.
Three out of eight children's immunization records reviewed. One child did not have an immunization record on file. Two other children did not have an up to date immunization record on file.
A small playhouse was observed to be broken and had sharp edges. An outside white board was observed to be broken, had sharp edges and was rusted. Corrected at inspection. The playhouse and the white board were removed from the playground during the visit.
The infant classroom had 8 children with one caregiver and a specified age group of 12-17 months they were over ratio by 3 children.
Four out of ten children's records reviewed were lacking health statements on file.

The infants were not taken outside to play, and outdoor play was not listed on the activity schedule.
The menu posted said they were serving bean and cheese tacos, but they served cereal, milk and apples. Corrected at inspection: The menu was corrected during the inspection.
The fire inspection posted at the center was last conducted on 04/12/2021 and is expired as of 04/12/2022.
One of the two passenger doors does not open.
The door trim on the inside of the door facing the inside of the infant room is broken and begun to splinter. The door trim on the outside of the door facing the playground is broken and starting to splinter. The privacy fence in the playground has three broken slates.
A caregiver was heard saying to a child that did not want to lay down, "Do you want me to call the cops?".
A high chair was observed in a Pre-K classroom and it is used to sit a 3 year old child that does not want to stay still to do activities. Corrected at Inspection: The high chair was removed from the classroom.
The wooden climbing structure has a broken plank that a child could get stuck by a splinter. A plastic toy tool bench in the playground was observed to be broken and has sharp edges.
The first aid kit in the transportation van contains aspirin. The first aid kit is not locked up and it is in reach of children.
