The center added 4 rocking cars into the playground and they were only 6 feet apart and not the required 12 feet a part from each other.

Four classrooms were observed to be out of ratio. One infant classroom had a specified age group of 12-17 months and 10 children in care. The classroon was out of ratio by five children. The second classroom had a specified age group of 0-11 months and had 6 children in care.They were out of ratio by one child. The third and fourth classroom had a specified age group of 2 years old and had 14 children, they were out of ratio by 3 children in each classroom.

The person left in charge did not know they were suppose to notify CCR about a child that was injured at the center and received medical attention.

The center was not allowing parents to go past the entrance into the classrooms.

The center failed to notify Child Care Regulation within 48 hours of an incident with a child that required medical attention.

The date for the fire drill, lock down drill, and severe weather drills were documented but the amount of time it takes to complete the drill was not documented.

A caregiver was observed to be outside of the classroom washing a child's hands and leaving the children unattended during naptime.

The Director did not complete the required 30 hours of annual training for the 2020-2021 training year.

The infant classroom was out of ratio. They had 5 infants between 0-10 months with one caregiver.
