It was determined throughout the course of the HHS investigation that caregivers failed to utilize good judgement. A bottle was warmed up in a bottle warmer for an infant in care. The guardian refused the bottle at pick-up. A caregiver stated to the guardian they would save the bottle for the following day which violated manufacture instructions on the formula can leading to potential harm of the infant in care.

A 5 month old was observed sleeping in a restrictive device. When the caregvier was asked how long the infant had been sleeping in the device, the caregiver answered 4 minutes. This was corrected at inspection when the caregiver was asked to remove the infant from the device and the infant was placed in their crib bringing the operation into compliance.

Lysol toilet bowl cleaner was observed in an unlocked cabinet in a bathroom that was accessible to children in care. Windex was observed in an unlocked cabinet in a bathroom that was accessible to children in care. Clorox wipes were observed on a table in the bathroom in the toddler room that was accessible to children in care. This was corrected at inspection when all of the supplies were removed and made inacessible to children in care.
