A play equipment was observed in the use zone of the exit of a slide in the playground area. The 6 feet use zone was not compliant. NOTE: The use zone in front of the exit of the slide was made compliant during the inspection. Play equipment was moved away from the 6 feet use zone of the slide equipment.
Infant group was excluded from practicing a fire drill during the month of August and September as per a caregiver and director of operation. The last completed fire drill was for the month of September 2023, but infants did not participated in the drill.
Two caregivers that transport children in the operation's vehicles do not have transportation training.
Based on evidence gathered during the investigation, use of prohibited discipline and guidance methods were practiced at the operation. An employee pulled a child to redirect, and a second employee slapped a child's hand/arm.
Admission information for a child was not available for review during the investigation inspection.

One caregiver was providing care of 6 infants with a specified age group of 0-11 months old. Child/Caregiver ratio was deficient. NOTE: This was corrected during inspection. A second caregiver was added to the classroom.
One caregiver was missing 3 hours of pre-service training to meet the requirement of 8 hours prior to being counted in child/caregiver ratio. A second caregiver was missing the orientation statement.
As per caregivers, cloth toys in the infant classroom are being machine washed every two or three weeks. Two of the cloth toys were observed to be in use during inspection.
Children's products certification was not completed timely. It was last completed on 8/31/2020.
