Outdoor area had a pile of trash, plastic knife, and empty can easily accesible to the children.

One classroom did not have activity plan available. Correted: This was corrected at inspection as director posted current activity plan.

Caregivers did not sanitize diaper changing area or change the non-absorbent paper liner between diaper changes.

The following trainings were not available for review for two caregivers: (1) Emergency preparedness. (2) Preventing and controlling the spread of communicable diseases, including immunizations. (3) Administering medication, if applicable, including compliance with 746.3803. (4) Preventing and responding to emergencies due to food or an allergic reaction. (5) Understanding building and physical premises safety, including identification and protection from hazards that can cause bodily injury such as electrical hazards, bodies of water, and vehicular traffic. (6) Handling, storing, and disposing of hazardous materials including compliance with 746.3425.

A 16 month old infant was in the same group of children as 3 year old children. Operation was deficient in the 18-month rule standard.

Emergency medical transport and treatment authorization forms for each child being transported was not present in the transportation vehicle used on this day.

Two caregivers were missing training hours documentation. One of the caregivers was missing additional required records.

Children's products certification was not completed timely. Last completed certification was completed on 7/7/2021.

A crib labeled with another infant's name was in use by an infant. NOTE: This was corrected during inspection. Infant was moved to their assigned crib.

Caregiver did not wash hands after diapering a child.

CPR and 1st aid trainings were not available for review for one caregiver.

The use zone of a rotating device was deficient as observed during inspection. Another play equipment and a table were less than 6 feet away from the rotating device. NOTE: This was corrected during inspection. The table and play equipment was moved to comply with use zone.

There were 21 children present. 17 children were signed in to care.

Relocation diagrams need to be posted in the school age and pre-k 5 classrooms.

The annual health inspection needs to be conducted under the new operation name.

The fire department needs to conduct a fire inspection under the new operation name.
