A 16 month old infant was in the same group of children as 3 year old children. Operation was deficient in the 18-month rule standard.
Emergency medical transport and treatment authorization forms for each child being transported was not present in the transportation vehicle used on this day.

The use zone of a rotating device was deficient as observed during inspection. Another play equipment and a table were less than 6 feet away from the rotating device. NOTE: This was corrected during inspection. The table and play equipment was moved to comply with use zone.
There were 21 children present. 17 children were signed in to care.

The annual health inspection needs to be conducted under the new operation name.
