Operation did not have a plan to handle transportation emergencies.

Two classrooms did not have an activity plan.

Classroom was observed to not have enough toys for all of the children. Children were observed fighting with each other for toys.

Children's hands were not washed after diaper change.

Two-month-old infant was observed swaddled in a crib.

No drinking water was available for toddler and school-age children during mealtime and during active play.

Childrens product certification was last completed on 1/10/2023.

Caregiver was observed not washing her hands after changing children's diapers.

A caregiver left three children unattended in the after school room when the caregiver stepped out of the room to take a child to the toddler room.

Two school aged children in the afterschool room were observed watching photos and videos on a cell phone. The television in the afterschool room was on playing a cartoon not related to the planned activities. The cartoon playing on the television was (Masha and the Bear).

The operation did not notify child care regulation after learning about a child being injured while in care and requiring medical treatment.

Based on preponderance of evidence gathered during the investigation, two caregivers have yelled at children in care at the operation.

A 6-month-old infant had a blanket inside their crib during nap time. Infant was awake. NOTE: The blanket was removed by caregiver upon my arrival to the room.

One infant was observed in the same group of children as an 11 year old child. In another classroom, a 20-month-old was observed in the same group as a 1-month-old infant. 18-month difference was exceeded. NOTE: This was corrected during the inspection. Children were regrouped and compliance was met.

Child/Caregiver Ratio was deficient in the infant classroom by 1 child. There were 2 caregivers with 11 children ages 1 month to 20 months old. Specified Age Group was 0-11 months old. NOTE: This was corrected by the oldest child being moved to another group. Child/Caregiver Ratio was met.

A new caregiver having unsupervised access to children in care did not have First Aid training.

Two activity plans posted in two classrooms in use were not dated.
