Hand washing did not occur before snack in the Pre-K classroom and before dinner for a group of children in the School Age classroom as observed during inspection.

During the investigation it was found that children were placed in high chairs as a form of punishment. The caregiver is no longer employed at the operation.

A caregiver at the operation has a high school diploma which is not valid.
