Two children's service plans were not reviewed and updated within the required time frame.

A child was not wearing a seatbelt when the vehicle he was being transported in by a caregiver was pulled over by law enforcement.

The foster home screening did not document if local law enforcement was contacted regarding any service calls to the home.

A caregiver placed a child at risk of harm when engaging in reckless driving, driving while intoxicated, and fleeing form law enforcement.

Two of six files reviewed had supervisory forms that were not signed accurately.

A child over the age of one year has not had a completed TB test.

During a walk through of the home a crib was observed to have a crib bumper and stuffed animals in the crib.

In one of seven child files reviewed, the child's service plan indicated the child was prescribed medication for a medical condition but the medications were not listed in the service plan.

A log was not being used to document unauthorized absences.

During a walk through of the home a crib was observed to have a crib bumper and stuffed animals in the crib.

The caregiver would have the child stand with her back against the wall as she squatted and to do pushups as a form of discipline, for bad behavior.

The caregiver bit a child as a consequence for the child biting the caregiver.

The caregiver threw away a jewelry box containing a photo of a child loved one.

The home has a damaged wall outlet, and a damaged light switch. The children's room has rodent dropping between the mattress and in the dresser.

a child in care had bed linen that was dirty and had not been washed.

Bed linens were consistently observed dirty over a month of children being placed. Foster parent stated relying on the children to bring down linen to be washed due to limited mobility in foster parent's left hand.

A child in care was pushed by foster parent as a result of an altercation between the child and foster parents grandchild. Foster parent also made inappropriate and unnecessary comments about food making children in care not wanting to eat. Lastly, foster parents adult daughter made an inappropriate remark to children in care resulting in children in care feeling threatened.

3 of 8 child records were missing TB tests.

No medication log was kept for youth in care.

Caregivers utilized inappropriate physical discipline and forced children in care to kneel on rice as a form of punishment.

A 6, 8, and 9 year old child in care were inappropriately touched by their caregiver.

Caregiver did not demonstrate self-control when engaged in inappropriate contact with children in care.

During the inspection cleaning chemicals were stored under the sink and pantry without being secured and our of reach of children.

During the inspection child's type 2 medication was found secured by only one lock.
