Observed documentation of last fire drill conducted on 1-31-24.

In the infant classroom there was a garbage can that was accessible to the children. NOTE: This was corrected at inspection when caregiver removed the trash can from the classroom.

In the toddler classroom there was an ointment in one of the children's cubbies that was not labeled. NOTE: This was corrected at inspection when caregiver labeled the diaper ointment.

A Pre-K child was observed using caregiver's phone to watch videos not related to planned activities. NOTE: Phone was removed and activities were resumed.

A caregiver was observed yelling at a child during the inspection.

Two groups of children were observed watching cartoons on television not related to planned activities. NOTE: This was corrected by caregivers switching TV shows to material related to planned activities.

Three out of nine infants were observed sleeping in restrictive devices (swings). Children were not removed as soon as possible. NOTE: This was corrected by caregivers removing children from restrictive devices.

A caregiver washed a child's hands in the sink designated to wash dishes and other food items.

Documentation of education outside of U.S. for one caregiver does not provide proof of high school or GED completion. Documentation of education outside of U.S. for another caregiver was not translated to English.

Three children under the age for 2 were observed watching television during a transition in the toddler classroom. NOTE: This was corrected during inspection as television was turned off and another activity was initiated.

An infant's bottle was being supported by a blanket while infant was feeding on a restrictive device. NOTE: This was corrected during inspection as infant began to be fed by a caregiver.

A play yard was observed without a fitted sheet in the infant room. Note: This was CORRECTED during the inspection as a fitted sheet was placed on the play yard's mattress.

Based on the information obtained during the investigation, it was determined the Director's presence at the center is not routine and adequate enough to meet the position?s responsibilities. Timesheets could not be provided to account for director's presence.

Additional matresses were observed in two play yards in the infant room. Note: This was corrected at inspection as the additional mattresses were removed.

An infant was not strapped in while on a swing. Note: This was corrected at inspection as the infant was strapped in.

A child was served their A.M. snack on a bare table. Note: This was corrected during the inspection as the snack was placed in a child's plate.

A television was observed on a children's table in a toddler room. The television was not anchored to prevent from tipping over. Note: This was corrected during the inspection as the television was removed from the room and stored in the office.
