Based on the information obtained during the course the investigation; caregivers were observed utilizing a cellphone and sitting - facing away from a group of children during naptime. An incident took place during naptime and went unnoticed, and incident required a child to receive medical treatment.

The center has decided to arrange nap mats with the green side as the sleeping side and the red as the floor side. During the inspection. Some of the mats were arranged with the green side the floor.

The transportation binder did not include emergency medical authorizations. These were added to the binder from the children's files at the time of the inspection.
Child/Caregiver ratio was reduced for a group of children during nap time. There were 14 children ages 12 months to 2 years old in the group of children. NOTE: This was corrected during an inspection. A second caregiver came into the classroom and child/caregiver ratio was met.
Screen time activities were observed in the school age classroom during rest time. Children were watching TV and two children were using their personal electronic devices. NOTE: This was corrected during the inspection. All the screen devices were put away and children transitioned to another activity.
Based on evidence gathered during the investigation, a child hit a child with a chair causing an injury to child's head. Another child was made responsible for causing the injury due to caregiver not providing supervision and was not aware of children's on-going activity.
Based on evidence gathered during the investigation, a child sustained an injury while in care and received medical attention. Child Care Regulation was not notified by the operation.
Attendance records for director and owner were not available for review during inspection.
Unitary surfacing was observed torn and unmaintained during inspection.
Two diaper changing table pads had tears on the surface making them absorbent. NOTE: This was corrected during inspection. Diaper changing pads were replaced for new ones.
Adequate lighting was not available in one classroom in order to provide visual supervision of children. NOTE: This was corrected during inspection. Lighting was adjusted to meet compliance.
Fire extinguishers were last serviced in March 2021. NOTE: This was corrected. Fire extinguishers were serviced during this inspection by an accredited company.
Parents of children enrolled were not notified of a child in care or an employee contracting a communicable disease.
Child/Caregiver ratio was reduced by 50% during nap time while having 5 infants present in the group of children. One caregiver was supervising children in two classrooms divided by a child gate. Two caregivers were needed for the group of children and it was reduced to one.
An infant was feeding from a bottle and had their bottle propped with a stuffed animal. NOTE: This was corrected during inspection as caregiver removed stuffed animal and bottle from infant.

A carbon monoxide detector was not present at the operation.
