A carbon monoxide detection system could not be located during inspection. This was corrected when a battery operated carbon monoxide detector was found in the office. New batteries were installed and a notation on Fire Safey Practices Form 7263 was added for monthly checking and servicing of carbon monoxide system.

The minibus used for transportation contained a fire extinguisher with an expired service tag dated March 2022. This poses a hazard to children because if a fire was to start on the minivan it could not be extinguished.

The outdoor playground structure bridge was cracked. The crack on the bridge has an exposed sharp edge that could be hazardous to children. Photos were taken and uploaded to operation's digital storage.
Operation failed to notify Licensing that they added infant care services. Person in charge stated they have been providing infant care since November 2022.
During walk through of the toddler room, a 55-inch television was not properly anchored/secured. The person in charge of the operation removed the television from the toddler room and placed it in a storage area that was out of children's reach.
Operation did not have files available for review for an 11-month-old infant and a 12-month-old infant.
The minibus used for transporting children contained an incomplete first aid kit. The kit contained expired multi-sized adhesive bandages. The kit contained loose disposable gloves deeming them unsterilized.
Six windows on minibus used for children transportation do not open. The minibus child alarm system is not in operation.
The operation was missing four required lock-down drills for 2022.
During review of records, a pre-school child was missing a written statement from a health-care professional indicating the child is able to take part in the child-care program.
During walk through of operation Inspectors observed an 11-month-old and 12- month-old children in the toddler room because there was no infant room set up. The 11-month-old infant is non walking and did not have a crib. Photos taken and uploaded to operation's digital storage.
During evaluation of records, three caregiver files were missing eight hours of pre-service training and the notarized affidavit.
The operation had a urinal in the boy's bathroom that was not operational and in need of repair. Person in charge stated the urinal has not been operational for the past six months. Photo taken and uploaded to operation's digital storage.
The outdoor playground area has aluminum siding that is rusted with sharp edges. There is a hazard of children cutting themselves on the sharp edges. Photos taken and uploaded to operation's digital storage.
The operation has a structure with a slide in the toddler room. The slide was observed cracked with clear tape however slide should not be used because the taped area is a weak spot. The legs of the structure were loos and not screwed in. Corrected during inspection. Entire structure was removed from the toddler room and placed in a storage close out of reach of children. The outdoor playground has a loose sand fill surface less than six inches. Photos taken and uploaded to operation's digital storage.
The outdoor play area had two pogo sticks, stilts and a shopping cart that can cause injury to children in care. Corrected during the inspection, the pogo sticks, stilts and shopping cart were removed from the outdoor play area and placed in an area out of the reach of children. Upon review of people list there were two individuals who have no longer been working at the center and had not been inactivated within 7 days of leaving the center. This was corrected when the director inactivated the individuals in the CCR account during the inspection.
The annual fees were due 2/16/22 and had not been paid. The director paid the amount due at time of inspection.
During inspection there were two classrooms that did not have daily schedules posted.
