A child in care was transported by the operation's staff and taken to the incorrect elementary school. While the child was supervised at all times, this was a non-routine situation that placed, or may have placed, this child at risk. This situation required that the operation notify CCR. The operation did not notify CCR as soon as possible and/pr no later than two days after the incident.

One of the exit doors in Building G was locked during business hours. This door functions as an exit during an emergency. This was corrected at inspection when the door was unlocked.
During inspection there was one caregiver supervising ten children, whose specified age group was 15 months. This was during nap time, however, the naptime ratio does not apply to children under 18 months. This was corrected at inspection when the second caregiver returned from lunch.
A van for more than eight passengers was purchased January of 2022 but was not equipped with an electronic child safety alarm system. Driver confirmed she has been transporting children in this van since it was purchased. Another van's Child Safety Alarm system was not in operation. Person in charge indicated the van was taken to get serviced and the alarm was disconnected. Person in charge confirmed the van has been used to transport children.
Several transportation vehicles had incomplete first aid kits. This deficiency was corrected when person in charge replaced missing items in first aid kits during this inspection.
A six-month-old infant was observed sleeping in his crib on his stomach. Infant caregivers confirmed infant was placed on his stomach because infant allegedly is able to roll over. During this inspection infant was placed on his back but did not roll over. Photo was taken and uploaded to digital storage.
The older infant room was observed out of ratio during the follow-up inspection. There was one caregiver for nine children with 17-month-old specified aged group. This was corrected during the follow-up inspection. Three 21-month-old children and one 19-month-old child were moved to the two-year-old classroom.
Fire extinguishers serviced and tagged on August 2020, were not serviced and tagged during the follow-up inspection. Person in charge stated she was informed by the service company they were short staffed but will expedite to get the extinguishers serviced and tagged ASAP.

Daycare driver dropped off a three-year-old child at the wrong school because the transportation roster was not checked to ensure the correct drop off location of the child. The child was never at risk or imminent danger because he was always under the care and protection of the elementary school staff.
It was determined the operation failed to use proper judgment when OV's mother called the daycare to ask the whereabouts of her child. The operation did not contact the driver to see where the child was at. It was determined child had been dropped off at the wrong school until the driver returned to the daycare.
Based on the information gathered during the investigation it was determined the operation failed to provide proper supervision when a two-year-old child placed her hand in the backside opening of the glass door while children were entering the building from the playground.
Operation failed to notify licensing of child's injury requiring medical treatment.
