This caregiver conducted transportation and did not know this child was in the transportation vehicle. This resulted in the child being unaccounted for and left alone and unsupervised in a transportation vehicle for two hours.

During a DFPS investigation, it was determined that a caregiver left three children alone in a van for approximately 10 minutes when she exited the vehicle to pick up additional children at a school. The van remained on when the caregiver left the children unattended.
During inspection, a caregiver was observed to be supervising children without cleared background checks, to include pending fingerprints and pending national sex offender registry checks. Caregiver was allowed to be present before receiving notification from the criminal background check unit that the caregiver was eligible. This was corrected when this caregiver was removed from the operation. Caregiver's fingerprinting appointmnet was scheduled for 5/9/2024 during inspection.
