During a DFPS investigation visit to the operation, the investigator observed some live and dead roaches. A photo of a dead roach was obtained.

In the infant room, the cribs were positioned in a way that allowed for the children to reach into the crib of another. This was corrected at inspection when the caregiver repositioned the cribs so the children could not reach and touch each other.
