During a DFPS investigation visit to the operation, the investigator observed some live and dead roaches. A photo of a dead roach was obtained.

In the infant room, the cribs were positioned in a way that allowed for the children to reach into the crib of another. This was corrected at inspection when the caregiver repositioned the cribs so the children could not reach and touch each other.

In the infant room, one infant who is not yet ready for table food, did not have any written feeding instructions available for review.

All of the caregiver's records that were reviewed were missing the required 24 hours of annual training. Director stated the staff had not yet completed the required training.
