Video footage was provided to CCR, where the driver is heard admitting to transporting a child in care without a car seat. Based on the child's age, weight and height, this child requires a car seat/booster seat.

The operation did not notify the parent immediately, as required. When the parent picked up their child that day, the operation still failed to notify them of the incident.
