In review of children's records, two children did not have health statements in their files or available for review.

In review of employee records, one employee working in the infant class did not have training in Sudden Infant Death Syndrome in her file or available for rewview. This was corrected at inspection when the employee completed SIDS training during inspection.

Operation did not have an emergency evacuation and relocation diagram posted in each room the children use. This was corrected at inspection when a caregier posted the evacuation and relocation diagram in each room used by children.

One child did not have emergency contact information in the binder while on a field trip. This was corrected at inspection when the director made copy of the emergency contact information portion of the child's admission forms and placed it in the transportation binder.

On the outdoor play area for toddlers, three slides had a use zone less than 6 feet on the sides of the slides. This was corrected at inspection when the slides were repositioned, creating a use zone of at least 6 feet on both sides.
