The operation is out of ratio in the following groups. The infant group ages 11 to 17 months has 6 children present with one provider. This group is over ratio by 1 infant. The 2 yr old room has 16 children with one provider. This group is over ratio by 7 children. The 4 yr old room has 21 children with one provider. This group is over ratio by 3 children.
The operation did not have a record of an incident report available for review during the visit/investigation.
The operation did not report to the local Licensing office or State Wide Intake an incident that involved a child in care that wandered away from the operation.
The operation staff released a child in care to an individual that was not authorized nor identified as a release person in the admission form.
An infant room provider was observed caring for 7 infants. Four infants were younger than 11 months and 3 infants were older than 11 months. Based on the ages of children present another provider was needed.
The transportation vehicles must have the interior areas vaccumed and cleaned. There was food, trash and other debris strewn throughout the floor and seats.
The carbon monoxide is not functioning at this time.
During the course of a DFPS investigation it was found that the operation had an outdoor play equipment that was not maintained. The play equipment had a gap/hole in it. This equipment was the cause of a child in care, sticking their finger in this gap/hole, which resulted with the child to sustain an injury that required medical attention.
