During the course of a DFPS investigation conducted on 7/26/21,trash bags were observed hanging on the outdoor play area chain link fence. Trash bags were noted to have tears and trash inside being exposed. On a follow up inspection was conducted on 7/28/21, it was noted that trash bags had not been removed. (Photograph Taken)
During the course of a DFPS investigation, chairs in the outdoor play area were observed to have broken back rests as well as the seating area. The rough/sharp plastic exposed poses a laceration and pinching hazard for children in care. (Photograph Taken).
During the course of a DFPS investigation school age children were observed using their personal electronic devices (cell phones/Nintendo Switch) playing video games and watching online videos which were not part of the daily planned activities that meet educational goals.
Two out of four employee files checked did not have the required annual training hours available for the previous fiscal year. Specifically one employee file was missing 9 hour for year 11/19-11/20 and another file was missing 15.5 hours for year 10/19-10/20.
A portion of the chain link fence located in the outdoor play area will need to be re-anchored/ secured. The top metal tube railings are loose and the fence is woobly; it could potentially fall on top of a child if it is pulled on. The metal tubes ends are exposed as well and are a laceration hazard. ------------------------------------------------------------------- A restroom will need to have the wood base board re-anchored as it is detaching from the frame. The wood board has seperated sufficient that a child's hand can get stuck in this gap.
A transportation vehicle will need to have the fire extinguisher serviced. The current service tag expired on 1/21.
