Transportation vehicle (minibus) front row window safety belt was missing a part that must be inserted into the buckle. Four backside seat rests locking latch were not in proper working order. Pictures were taken and uploaded to digital storage. This minibus will not be used to transport children until it has been repaired.

Van #8, the only van with a child safety alarm system did not function during this inspection. Driver assisted with the testing of the alarm and was not able to activate the alarm after several attempts. Children will not be transported in this vehicle until the child safety alarm system has been repaired.
Vehicle documentation verifying proof of purchase was not available for review for bus C26.
A child was left unattended in a transportation van for approximately 5 minutes after being overlooked by the caregiver in charge of transporting the children.
A caregiver was neglectful by failing to ensure all children exited the transportation van, resulting in a child being placed at risk of harm.
The operation failed to make a report to Child Care Regulation after becoming aware that a child had been placed at risk while in their care.
An incident report was not completed for an incident in which a child was left unattended in a transportation van placing him at risk of harm.
It was determined that a caregiver failed to use good judgment when she failed to attend to a child's needs after he was injured and exhibited signs of pain and discomfort.
The operation failed to generate an incident/illness report for a child that required medical treatment.
Operation failed to notify Child Care Regulations in a timely manner of a child who was injured and received medical treatment. Parent notified the operation on 04/08/2022, the operation notified Child Care Regulations after hours by leaving a message on the intake line on 04/11/2022.
During the DFPS investigation, it was determined that neglect occurred with a child in care who was seriously injured, resulting from the caregiver's inadequate supervision and conscious disregard for the safety and well-being of the children in care.
During the investigation it was determined a caregiver failed to exercise proper supervision by leaving the toddler classroom unattended several times.
During the investigation it was determined a caregiver was using her personal cell phone while caring for children, as a result a child was severely injured.
Based on the video footage obtained through the investigation, children were observed to be out of control, wrestling with one another, hitting each other in the face, and running out of the toddler classroom while a caregiver was on her cell phone.
A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.
Former caregiver confirmed she was present at the operation in care of infants on Monday 12/06/2021, Tuesday 12/07/2021, and Wednesday 12/08/2021, Caregiver resigned Wednesday night 12/08/2021. Caregiver was not present during the investigation on 12/13/2021, however CLASS indicated caregiver had a PENDING status. Director, designee, and an infant caregiver acknowledged former infant caregiver's presence at the operation on the above-mentioned dates. Former caregiver eligibility letter was received by the operation on 12/15/2021.
During the investigation/inspection a six-month-old infant was observed sleeping in a restrictive device (bouncy). Corrected during inspection when infant caregiver moved infant to a crib. (Photo taken).
