During inspection, one bottle of milk was observed to have no name or label to identify the child to whom it belongs. This was corrected at inspection when the caregiver labeled the bottle with the child's name.

The annual Child Product Safety Commission form was not available for review. This was corrected at inspection when the director completed a new form.

The operation fail to give a copy of the operation policy to the parents.

Two children were not given a quiet activity after waking up from nap. This was corrected at inspection when caregiver gave the children a book and quiet toy.

One infant feeding schedule had been updated in the last four months. The last updated was 11/21/2021.

There was a lack of loose fill surrounding the play-scape area outdoors and the ground beneath was visible.

There was no activity plan posted for the infant and pre-k classrooms. NOTE: This was corrected at inspection by director posting both activity plans.

Four out of four children's records were observed to be lacking one or more of the following: emergency contact address, physician information, special needs statement, enrollment signature and health statement.

The director has completed 4 of the required 30 annual training hours towards the 2020-2021 training year.
