One caregiver was seen sanitizing equipment and vaccuming while three children were present in the classroom.

One of the two outdoor gates were locked and the employee did not have access to a key to unlock it in case of an emergency. This was corrected at inspection when a copy of the key was given to the caregiver and added in the list of things to bring outside with children.

The operational policies were missing section 18 subsections A, B, C, D, and F.

One of the six cribs used in the infant classroom was recalled in January 2010. At the time of inspection, an infant was sleeping in the crib. This was corrected at inspection when the crib was removed and disposed of. The operation does not need the crib for the time being due to the enrollment in the classroom.

One infant was observed sleeping in a swing with a blanket. This was corrected at inspection when the child was moved to a bare crib.

One classroom was not being supervised during naptime.

A caregiver was observed not washing their hands between each diaper change.

Infants hands were not washed after diaper changes.

The operation did not inform Child Care Licensing of children being placed at risk within two days of the incident occurring.

Staff were told to delegate reports of abuse and neglect to another person. This was corrected at inspection when the director changed the operational policies and informed staff that reports cannot be delegated to another person.

Three out of six records reviewed did not have a health care statement or signature from the parent that the child has seen a doctor within the last twelve months.

Four out of six records reviewed did not have a parent's authorization to obtain emergency medical care.
