Florence Head Start

A infant was sitting in a small highchair that was placed on the floor. The highchair was missing the straps for the infant to be secured in the seat. This was corrected when the infant was removed from the seat.

The operational policies were missing the parents rights section. I spoke with the director regarding how to add this policy and to have the parents resign the new policy.

Observed a climbing equipment with slides that did not have 6 feet of use zone from the bottom of the slide to the building and from the stairs to the fence. I spoke to the director about this.

Observed the operation to have a new toddler playground. The operation failed to communicate with licensing to let them know of the addition of space. I spoke to the director about notifications and measured the new space.

Observed no fire drills being completed in September or October 2022. This was corrected when a fire drill was completed.

Observed a metal vent that is broken and has sharp metal sticking out which could cause a child to be scratched or cut by the metal.

The children product safety report had not been completd since 2019. This was corrected when another was completed during the inspection.
