A caregiver left a child outside on the outdoor playground alone. The child was outside approximately 11 minutes. The caregiver did not have auditory or visual awareness of the child.

This standard was evaluated as a result of a DFPS investigation and found to be deficient a child in the care at the operation was neglected at the operation.

A caregiver was not competent in following the operational procedures for account for children during transitions. The caregiver marked a child as in the room when the child was not in the room. This let to the child being unsupervised outside.

The operation did not have a current gas leak test on file for review.

The operation did not have a current fire inspection the most recent one was dated on 10/11/2022.

This standard was reviewed in part of a report and found to be deficient. A child was found unsupervised on a playground.

The operation did not notify CCR timely when a child needed medical attention.

The operation did not report to licensing a situation in which a child was disciplined inappropriately and was possible abuse.

A caregiver yelled and talked aggressively to children as a form of discipline.

A caregiver pinched/grabbed a child's neck as a form of discipline. This act led to bruising to the child's neck.

A child was abuse while in care at the operation.

A caregiver was present assisting a classroom without a cleared background check. The caregiver left the operation at the time of inspection.
