The child in care did not have a life jacket on while in the swimming pool.

The caregiver failed to follow a child's service plan regarding supervision around water.

This standard was found deficient as part of a DFPS Investigation.

The four monthly contacts reviewed did not appear to be sufficient enough to observe the child' placement needs. The contact also did not document enough sufficient information.

The two quarterly contacts reviewed did not document the names of the household members who were present during the visit. Also, both foster parents did not sign the quarterly inspection form.

The medication record was not updated for 18 days in the month of February when medication was administered to the child in care.

Two of the four homes reviewed did not have a designated place documented to evacuate to.

There was a box full of medications not locked up.

During the walk through of the home, an infant's crib was observed to have the following inside the crib: pillow, blanket, toys, and a bumper pad around all sides of the crib.

Several serious incident reports did not have the foster home physical address or telephone number documented.

A child had a substantial critical injury that was reported 32 hours after the incident.

There were 21 serious incidents that the foster parent did not report directly to the hotline but they were reported by the agency.

The supervisory form did not document any changes in household members, major life changes or the foster home disaster/emergency plan as well as the challenging behaviors for children in care or stress levels for caregivers.

Two of the foster home files did not have a quarterly visit conducted timely.

Foster parent left a bowl of hot noodles on the kitchen counter and accessible to a child in care; and the child pulled the bowl down causing second degree burns. Foster parent was advised and provided a child safe gate to create a safe environment, but required consistent prompting before follow through.

A child in care did not receive follow up treatment timely and as recommended by a medical professional.

A foster parent hit a child in care on his back with her hand. The foster parent also spanked two children in care.

Employee did not have enough training hours in her file. Employee is needing to complete 4 hours of training.

Weapons without trigger locks were stored in the same locked box as the ammunition. Note: This was corrected at inspection due to the weapons and ammunition being separated into two different locked boxes.

A child in care had a burn on his arm and the foster parent is unaware of how it happened.
