The administrator was observed preparing medication for the children more than an hour before the medication was administered.

The medication log was reviewed during the inspection on 1/16/25. The medication log was last updated on 1/12/25.

A staff member did not follow the agencies policy regarding restraints.

Staff were witnessed sleeping when they were supposed to be supervising the children.

A staff put a child in a restraint when they tried to run away.

The two CPR and first aid training that were reviewed did not have the trainers qualifications. It also did not have the length of hours 748.915(b)(6).

Two children's file that were reviewed did not have the information documented regarding the contraindications restraints.

There were multiple prescription medications stored in a zip lock bag.

A caregiver did not complete the EBI training within 6 months of the previous training date.

There were multiple certificates that did not have the length of the training.

During the investigation, information was obtained, that two children in care engaged in hitting one another in three different occurrences before staff members were made aware.

A current staff member s background check was inactive as of 5/16/2023.

A staff member did not implement recommended techniques for discipline when engaging a child in care.

Two staff files did not include documentaiton of total required pre-service training hours.

One staff did not complete EBI pre-service training requirement hours.

The service plan for one out of two child files was not signed by the child.

Three employee records were reviewed and two employees had pre-drug screening after their hire date. The hire dates that arevreported in the file are 10/22/2020 prescreening drug test 07/11/2021 and 07/17/2021.

There is no controlling person form for the administrator. The administrator date of hire was 10/22/2020.

Professional level staff must have the education and work experience to perform their job duties

Staff must have TB test or chest ex-ray before working with children in care and provide documentation.

One employee file reviewed did not contain verification that the employee met the educational requirement

A copy of the child"s orientation was not found in the file.

A record for the child to be able to commincate with his/her siblings.
