At the time of inspection the loose-fill surfacing (mulch) on the playground was observed to be less than 2 inches under climbing equiipment.

This standard was found deficient as part of a DFPS investigation. The training records for a caregiver were not available during the visit to the operation by the DFPS investigator. The training records were requested multiple times by the Investigator before they were received.
This standard was found deficient as part of a DFPS investigation. A caregiver did not demonstrate patience with a child during naptime. This is evidenced through the caregiver?s actions towards the child such as pointing her finger in the child?s face, patting the child roughly on the back, and aggressively placing the child on a nap mat.
This standard was found deficient as part of a DFPS investigation. A refrigerator was not secure and allowed children under the age of two to open the refrigerator; as a result, a child got injured.
Evidence shows that a school age caregiver required the children to sit down on the carpet for at least 10 minutes or longer before engaging with the children. The caregiver did not give the children an activity or specific instruction.
At the time of inspection, children in the school age classroom were observed watching "Jumanji". One child was observed taking a nap on one side of the room. A couple other children were observed laying down, but they were not asleep.
There was evidence found proving that the operation was out of ratio while on a field trip. There were two groups of children on the field trip. The ratios of the groups were 12:1 and 14:1. The youngest child on the field trip was 6 years old. The 14:1 group was over ratio by 2 children.
Based on information obtained through investigation it was found that a child was left inside the classroom while the rest of the class had gone to the outdoor play area. The child was left inside for less than 5 minutes.
It was found that a caregiver did not notify or report to administration or parents when a child was left inside the classroom during a transition. It was also found that multiple caregivers had concerns regarding child safety and did not report this appropriately to administration.
This standard was evaluated based on a reported incident. Evidence shows that the caregiver's pediatric first aid training was expired at the time of incident.

Based on information gathered from a DFPS investigation, it is determined that the operation did not report the allegations within 48 hours to Licensing. The director was informed of the allegations on 8/15/2022 and Licensing received the report on 8/18/2022.
