The caregivers did not wash the children's hands after changing their diaper. This occurred in multiple classrooms.

The operational policies did not have a procedure for applying insect repellent or sunscreen.

6 of the 10 children's records that were evaluated by licensing were not completed nor maintained.

The operation did not have the transportation drivers personnel file on record for licensing to review.

The operations policies did not contain a promotion of or a time frame for indoor and outdoor physical activities.

The operation did not have emergency medical transport nor treatment authorization form for one child being transported on the bus.

The safety straps on infant feeding table were not buckled while in use by children.

The Television in PreK 4 classroom was not secured or anchored to a wall.

A caregiver did not have physical proximity to children while they were in the restroom, as a result it was an inappropriate interaction between children.

In a group of children the youngest child was 14 months old and the older child was 36 months, the children were 22 months apart.

It was observed the two toddler groups out of ratio. It was observed 13 children ages of 2 years old to 3 years old with one caregiver. It was observed 14 children ages of 14 months to 3 years old with one caregiver.

It was found the toddlers room to be over ratio when the operation adjusts ratios when combining children under 17 months old with children over 18 months old.

Infants were observed sitting on a swing for more than 15 minutes.

Shelves were observed empty during the inspection, toys and supplies were not available to the children. The toys and supplies were inside locked cabinets.

Caregivers did not have accessible, electronic or paper information about the children they were responsible.

Director did report and incident that involved a parent concern regarding mistreatment of a child. Follow Up 9/16/2022: Director understood the importance of reporting incident that placed a child at risk, however during the follow up inspection all deficiencies were not back in compliance.

Children were observed playing with balls while walking up and down the slides.

Caregivers unable to provide information of the children under their care. A caregiver was unable to tell names and age of the children. The infant teacher provide a number of children under her care, when counted again the caregiver it was an extra child in her room that put the caregiver out of ratio, Follow up 9/16/2022: Caregivers did not know the number of children and age of the children they were responsible.

It was observed three bouncers in front of infant playpen while the infant was not asleep, the caregiver was not able to see the infant.

Toys in the toddler room were not accessible to the children.

The operation did not report an incident to licensing that put a child at risk.

A caregiver asked another child to find a child who was missing.

A child was unsupervised when he was hiding from operation staff for an extended amount of time.
