When the child returned to the classroom, the caregiver learned that the child had fallen and hit their eye. The injury caused the child's eye to become swollen and bruised. The caregiver failed to intervene and ensure that the child was wearing shoes.

The Pre K (Fours) and the Tods/Twos classrooms were out of control. The children were observed jumping off classroom chairs, climbing on the play kitchen, screaming, running throughout the classroom, and not listening to the caregivers.
A chair with exposed staples was located in the infant/toddler classroom. The chair was accessible to children.
