The operation was not documenting substitutions to the daily menu.

Based on interviews it was determined a child was left outside unattended for less than two minutes. This was corrected at inspection when the director talked with both caregivers involved and discussed the transition policies and procedures to make sure the caregivers are following them at all times. The director reminded the caregivers to ask for assistance if needed.

Five infant feeding instructions were not current.

Out of ten files reviewed, two children were not current on immunizations.

Loose fill on two playgrounds were not at the required depth and needed to be maintained.

It was determined through interviews that a caregiver would remove a child from a meal/snack as a form of discipline, and would not allow the child to finish its meal/snack.

It was determined through interviews that a caregiver hit a child on the hand and bottom as a form of discipline.
