2 caregivers did not have required form 2912 on file. This was corrected during inspection with the caregivers filling out the form and the director placing them in the files.

Based on information gathered during the course of an investigation it was determined that a caregiver did not have visual awareness or knowledge of the ongoing activities while children were playing with scissors and therefore was not able to intervene when a child's hair was cut. Note: The incident occurred in August of 2023 and the operation has implemented new procedures regarding supervision since then.
