Based on evidence gathered from a DFPS investigation, it is determined that caregivers did not exercise good judgement by failing to report inappropriate discipline and mishandling to children by a co worker. Caregivers stated that they witnessed this concerning mishandling on multiple occasions, and did not report their concerns.

At time of inspection, two nap mat covers had cracked corners. The corners were plastic and had sharp edges. The cracked plastic also posed a pinching hazard. There were two tricycles on the playground that were broken. One tricycle had a broken pedal and the other had broken plastic pieces with sharp edges. Photos were taken of all equipment. The center director removed all broken equipment during inspection.
